Key Responsibilities

• Provide comprehensive care to patients, including consultations, diagnosis, treatment plans, and follow-ups
• Work in partnership with a multidisciplinary team, including nurses, pharmacists, and healthcare assistants
• Support patients with chronic disease management and preventative healthcare initiatives
• Offer health advice, lifestyle counselling, and disease prevention strategies
• Actively participate in team meetings, case discussions, and ongoing professional development
• Contribute to the improvement and evolution of practice processes and patient care protocols
