Clinical Pharmacist – York

Role Responsibilities:

• Optimize medication use for patients across GP practices in York
• Provide expert advice on medicine management and contribute to medication reviews
• Work alongside multidisciplinary teams to deliver high-quality care
• Participate in continuous professional development and clinical audits
• Engage with patients to improve their medication adherence and outcomes

Requirements:

• GPhC registered pharmacist
• 2+ years of experience in clinical pharmacy, ideally in primary care
• Excellent communication and teamwork skills
• A passion for improving patient care and outcomes
