Role Responsibilities:

• Provide clinical pharmacy support across multiple services, including medicine management, prescribing advice, and patient consultation
• Work collaboratively with GP practices, multidisciplinary teams, and community services to enhance patient care
• Participate in medication reviews, audits, and the optimization of therapy
• Support prescribing and ensure adherence to clinical guidelines and protocols
• Offer training and support to healthcare professionals on pharmacy-related matters

Requirements:

• GPhC registered pharmacist
• Experience in clinical pharmacy within primary care
• Excellent communication and team-working skills
• Ability to manage a demanding caseload effectively
• NHS experience is preferred but not essential
