Duties and Responsibilities:

• Conduct medication reviews and provide clinical advice to healthcare professionals.
• Collaborate with the multidisciplinary team to ensure safe and effective prescribing practices.
• Support patients in managing chronic diseases through medication optimisation.
• Deliver training and mentorship to other healthcare staff.

Qualifications:

• MPharm degree and registration with the General Pharmaceutical Council (GPhC).
• Experience in clinical pharmacy or hospital settings preferred.
• Strong communication and organisational skills.
