Clinical Pharmacist Job – Bath

• Conduct medication reviews to ensure safe, effective, and appropriate prescribing.
• Provide expert advice on the use of medicines to patients and healthcare professionals.
• Support chronic disease management through medication optimization.
• Contribute to clinical audits and quality improvement initiatives.
• Collaborate with multidisciplinary teams to enhance patient care.

Qualifications:

• Registered as a Pharmacist with the General Pharmaceutical Council (GPhC).
• Proven experience in a clinical setting, preferably within primary care.
• Strong communication and decision-making skills.
• Ability to work autonomously and as part of a team.
