Key Responsibilities:

• Deliver comprehensive clinical pharmacy services across a network of GP practices.
• Collaborate with healthcare professionals to optimize medication use and improve patient outcomes.
• Conduct medication reviews, provide prescribing guidance, and manage complex medication regimens.
• Contribute to the development and implementation of clinical pathways and protocols.
• Participate in audits, clinical governance activities, and service improvement initiatives.

Essential Requirements:

• GPhC registration
• Minimum 2 years of clinical pharmacy experience within a primary care setting.
• Excellent communication, interpersonal, and organizational skills.
• A strong commitment to patient-centered care and improving healthcare services.
