Clinical Pharmacist – Bradford

We are seeking a skilled and proactive Clinical Pharmacist to join our team in Bradford. In this role, you will contribute to improving patient care by providing expert guidance on medication and supporting multidisciplinary teams.

Duties and Responsibilities:

• Conduct detailed medication reviews to ensure safe prescribing.
• Offer expert advice on the use of medicines to patients and healthcare professionals.
• Support the management of long-term conditions through effective medication plans.
• Collaborate with GPs and other healthcare providers to enhance patient outcomes.
• Assist with clinical audits and service development.

Qualifications:

• Registration with the General Pharmaceutical Council (GPhC).
• Experience in a clinical pharmacy setting.
• Excellent communication and decision-making skills.
• Ability to work both independently and within a team.
