Clinical Pharmacist Job near Stoke-on-Trent

We are seeking a highly motivated and experienced Clinical Pharmacist to join a dynamic healthcare team near Stoke-on-Trent. You will play a crucial role in providing pharmaceutical care, optimising medication therapy, and contributing to patient safety. This role offers the opportunity to work collaboratively with multidisciplinary teams and make a real difference to patient outcomes.

Key Responsibilities:

• Provide clinical pharmacy services to patients, including medication reviews and reconciliation.
• Work collaboratively with doctors, nurses, and other healthcare professionals.
• Ensure compliance with relevant legislation and guidelines.
• Deliver patient education and counselling.
• Contribute to the development and implementation of clinical pharmacy protocols.

Requirements:

• Registered Pharmacist with the General Pharmaceutical Council (GPhC).
• Proven experience in a clinical pharmacy setting.
• Excellent communication and interpersonal skills.
• Ability to work independently and as part of a team.
