Clinical Pharmacist – Canterbury

We are seeking a Clinical Pharmacist to join a dedicated healthcare team in Canterbury. This role involves working collaboratively with GPs, nurses, and other healthcare professionals to ensure optimal patient care.

Key Responsibilities:

• Manage and review patient medication plans.
• Provide expert advice on the safe and effective use of medicines.
• Conduct medication reviews and reconcile medicines post-hospital discharge.
• Educate patients on proper medication use and potential side effects.
• Support GPs in achieving QOF and prescribing targets.

Requirements:

• Qualified Pharmacist with GPhC registration.
• Strong communication and organisational skills.
• Independent Prescribing qualification (desirable).
