Clinical Pharmacist – Reading

We are seeking a proactive and skilled Clinical Pharmacist to join a lovely team in the picturesque Reading region. In this role, you will play a vital part in ensuring the safe and effective use of medications, working alongside an integrated healthcare team to enhance patient care, improve therapeutic outcomes, and support overall medication safety across their services.

Key Responsibilities:

• Review and optimise medication regimens for hospital and community patients.
• Provide expert advice on pharmaceutical care and medication safety.
• Collaborate with multidisciplinary teams to support treatment decisions and care plans.
• Lead medication reviews, particularly for elderly or high-risk patients.
• Contribute to audits, quality improvement initiatives, and policy development.

About You:

• MPharm degree and registration with the General Pharmaceutical Council (GPhC).
